As each library has its own unique programs with a range of desired outcomes, there is no standard survey. This example survey provides guidance around a selection of questions you might consider, including outcome dimensions (such as learning, belonging, and access), open text feedback and demographics.
Distribution frequency
We recommend you distribute this survey at the conclusion of each event/activity. Distribution methods include via iPads or computers in the library, via QR code or URL links to complete on attendees’ own smart devices, or via email to registered attendees.
